How do I get a copy of my incident report?
  • You can obtain a copy of your incident report through your defense attorney. If you are not represented by counsel, you may request a copy of your report by filling out a GRAMA form in the police department office or by emailing your request to lynsey.zeeman@ephraimcity.org. After records receives your request, they will email you a GRAMA request form. Simply fill it out and email the GRAMA request form back to records, along with a copy of valid picture ID. Generally, reports will not be released until the case has been approved by a supervisor and closed. This can take a few business days up to several months depending on the case. We prefer incident reports be picked up in person. Incident reports will not be released until fee is paid in full.
